PRACTICE NEWS

Please note: If you have any communication needs and wish to access information in a different format please notify a member of Reception.

 

 We are now providing an electronic prescribing service.  If you nominate a preferred local pharmacy we can send your prescriptions directly to them electronically.  The advantage of this service is that you will not have to collect the prescription from the surgery, simply collect the medication from your pharmacy.  If you are interested, please go to your preferred pharmacy and they can arrange.

 

We now have an on-line service for making appointments and ordering prescriptions at the surgery.  If you are interested in this service and have not already registered, please speak to the receptionists.

 

PATIENT PARTICIPATION GROUP

This group is made up of patient representatives, general practitioners and management staff.  The group has now met three times and are in the process of discussing the views put forward from the questionnaires at the PPG meetings to come up with an action plans.

PATIENT NHS CHECKS

You may be entitled to an NHS Health Check please check with Reception.

This involves an appointment with the Practice Nurse who will record medical details, check height, weight, blood pressure and relevant blood tests.  She will then review your case following the results.

register_now_blue_signTo register at the surgery you will be asked to provide your current medical card or photographic identification in the form of a passport or driving licence and 2 utility bills for the address you are living in at present. Please ask at reception for a registration form to complete. 

You will be asked to complete details on the form. If you have any  difficulties with this please ask the receptionist for help. You may need to provide your National Health Number but note that this is not the same as your National Insurance Number. You can obtain this number from your current GP.

A health check appointment with the practice nurse will be arranged if required. This is to obtain as much clinical detail as possible and you may also need to see the doctor if you require medication. Your records will then be applied for but as these may take some weeks to arrive at the surgery it is very important that you attend for the health check if you are asked to do so.

When registering with the practice you will be allocated a named GP who is responsible for your overall care. A member of the reception team can inform you about your named GP. However, this does not prevent you from seeing any other GP of your preference.

New Patient Registration. The practice may ask you to register elsewhere if we feel that we cannot provide you with the services you personally require because you live outside of the practice area (see below).
